Medical Staff Application

2024 Dates: August 14th-21st Application Deadline: May 1st

Camp Happy Times Volunteer Information - Medical Staff

Eligibility
It is the policy of Camp Happy Times that each participant must meet the defined eligibility criteria. Administration, Counselors and Medical staff should be knowledgeable of and understand all eligibility criteria required for their participation in the Camp Happy Times.

Staff Eligibility Requirements

• Be licensed or have had applied for a Pennsylvania Medical License as a Registered Nurse, Resident, Nurse Practitioner or Physician
• Adhere to all Camp Happy Times Policies and Procedures
• Be willing to communicate with fellow counselors and staff effectively
• Complete the screening procedure successfully
• Agree to attend the counselor trainings as required
• Have no prior criminal record

Time Commitment

• Respond promptly to any correspondence relating to Camp Happy Times, pre and post.
• Arrive on time for scheduled Camp Happy Times events. Counselors/Staff are scheduled to arrive on August 14th, 2024 by 2:00pm. 
• Respond to application deadlines.

Application

• Completed applications must be returned by April 15, 2023.
• Provide a photo copy of the following with a completed application:
• Health Care Coverage – Front and Back Copy
• Driver’s License or State Issued Identification Card
• Valid Pennsylvania Medical License and Malpractice Insurance or Copy of application

Greetings from The Valerie Fund’s Camp Happy Times! We are excited to hold our 42nd camp session.

Camp Happy Times 2024 will begin on Wednesday, August 15th and end on August 21st.
                 
Again this year you will complete and submit your application online for The Valerie Fund’s Camp Happy Times.  Kindly take the time to complete all of the sections and the code of conduct. Please make sure that the areas marked with an arrow are electronically signed.

The application consists of two parts: LIT Application & Medical Application. 

• LIT Application (Part I) - Please complete Part I online.  You can then print out the Medical Application or request that a copy be mailed to you.

• Medical Application (Part II) - Call your doctor and schedule an appointment to have Part II completed and have them return it to The Valerie Fund office.

A critical portion of a making camp safe and successful for each of our LITs is to ensure that we have all of the proper paperwork from each camper and necessary volunteers/staff for camp.  You can help us by simply completing each portion of the required paperwork in a timely manner so that we can arrange the rest!

Please let us know via phone or email if you are planning on taking the bus to and from camp so that we can count on you to help chaperone our campers.

The application consists of two parts: Camper Application & Medical Application. Please make sure that the application is signed in the areas indicated with an arrow.

Application Deadlines and Mailing Address

Camper Application (Part 1) - Please complete Part I 

Medical Application (Part 2) - Call your doctor and schedule an appointment to have Part II completed and have them return it. There are instructions on the form for medical changes after submission. 

The 2024 application is available online this year for completion.  It will only take a few minutes to complete your Camper Application online and you can indicate if you will print out your medical application or if you would like one sent to your home address.
